How to apply... Follow these simple steps to submit an application for your agency:
Step 1: Download and print the application.
The free Adobe® Acrobat® Reader® is required
to view and print the application form. To obtain the free Adobe® Acrobat®
Reader®, click the button below.
 Step 2: Complete the application.
The agency application form must be completed in it's entirety. Failure to fully complete the application may result in delays in the approval of your application.
Step 3: Fax the completed application.
Fax the completed application to (818) 610-2066. You will be notified once your application has been reviewed.
